Fragmentation of the database, cache and vault and logs itself can be a major issue on top of the other points raised, best pratice would be separate these on a seperate disk "slices" under nt and perform defragmentation during off hours of work.
Also not writing anything else to these volumes if key to success as well.
I would suggest a volume for vault+ cache, one for the db itself and another for attachments and yet another for logs for the optimum setup.
